Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. If you have x-rays from a previous or referring dentist, please email or text them to us prior to your appointment. Please download and complete the Patient Info and Medical forms below and bring them to your appointment.
We are doing everything we can to keep you safe and healthy during your visit. This means implementing special COVID-19 procedures to safeguard both our patients and team members.

We are committed to providing excellent dental care at a fair and reasonable fee. Our staff would be happy to discuss any fees or financial issues in advance or at the time of your visit. We will make every effort to work with you to file insurance claims and to timely resolve any outstanding balances. The following is a statement of our Financial Policy.
Insurance: Insurance coverage is a contract between you and your insurance company. While we make a good faith attempt to verify coverage, we are not able to guarantee that the information given to us by your insurance is correct. It is your responsibility alone to know what services may or may not be covered by your insurance. We encourage you to refer to your benefits manual if you have any questions about covered services. You will be responsible for payment of all non-covered services.
Demographic Information & Insurance Cards: It is extremely important that we have updated demographic data so that we will be able to contact you in the future. We also must have a current copy of your insurance card and a photo ID on file at all times. If your insurance changes, it is your responsibility to inform us of your new coverage and to provide effective dates of your new insurance. If we do not have your updated insurance information and your claims are denied by your insurance company due to untimely filing, then these claims will become your financial responsibility.
Co-pays, Co-insurances, & Deductibles: Co-pays, deductibles and coinsurance are due at time of service. You are responsible for any balance not covered by your insurance company.
Non-Covered Services: It is possible that your insurance company may not cover certain procedures or treatments for certain diagnosis. Please be aware that you will be responsible for any non-covered services.
For your convenience, we accept Cash, Checks, MasterCard, Visa, and Zelle for payments.
As a courtesy to other patients, the dentist and hygienists, please call us if you are unable to keep your appointment.
